Last chance to fill out 5HF survey!

The Five Healthy Communities have responded well to this year’s survey, but with just a little more than one and a half weeks to go … we’d like to get as much community participation as possible!
Responses to the 5 Heathy Towns survey help the 5HF Board of Directors, coalitions, and communities determine how our resources are invested within our local communities. Anyone over the age of 18 and living within the 5 Healthy Towns service area (Chelsea, Dexter, Grass Lake, Gregory, Manchester, Munith, and Stockbridge) is encouraged to take the survey! Rest assured your responses are anonymous.
In addition to helping our communities, anyone who completes the 5HF survey can enter to win gift cards to local businesses or memberships to one of the 5 Healthy Towns Wellness Centers. Prizes are valued at at least $200 each, and there will be one winner chosen from each of our five school district areas.
