5HF asking residents to take short survey

The 5 Healthy Towns Foundation is conducting a survey that they and their partners will use for goal setting, strategic planning, and determining how funding is used in our community and region. If you live in the 5 Healthy Towns region made up of the school districts of Chelsea, Dexter, Grass Lake, Manchester, and Stockbridge, and are over 18, they ask that you take the survey. The survey will take fewer than 5 minutes to complete and can be found at www.5hfsurvey.com. All responses are anonymous and 100% confidential. As a thank you for your time, you can enter to win great local prizes after completing the survey, with prizes drawn every two weeks.
