Chelsea Hospital is asking Manchester area residents to take the time to complete this 2026 community health needs survey. Your answers help shape our local health services. It will take 10 to 15 minutes to complete. Participants must be 18 years of age or older and will be entered into a drawing for one of two $100 gift cards.
